This is most commonly caused by the flush valve or syphon not opening, or the cistern not filling.
Your toilet not filling is generally an issue with the ball valve which can be side or bottom entry. If it is stuck closed then it will not let any water into the toilet cistern, this could be caused by scale or wear.

This is a problem where the ball valve isn't closing properly and therefore allowing water to drip into the cistern.
Constantly running water into the toilet bowel is caused by the flush valve or syphon not closing fully and allowing more water into the cistern than it holds.
